- To complete the DSPS intake, you will need your Student ID (W number), which you would
have received after completing your CCCApply application to Chabot College)
- In the form you will be presented an opportunity to upload your disability verification
at the end.
- If you do not have it ready, you can bring it directly to our office, or email cc-dsps@chabotcollege.edu to submit it electronically after submitting the intake packet.
- Disability verification is most often an IEP or 504 plan, but can also be a letter
from a doctor.
- After we have your intake packet and disability verification, you can contact us to
make an appointment with a DSPS counselor.
- In your appointment your counselor will review any other steps needed to complete
the onboarding process.

A key difference between K–12 and college: In higher education, the student is the primary decision-maker for academic support services.
The Chabot College student is the DSPS client. Parents and other supporters are welcome to attend DSPS meetings with the student present; however, DSPS does not meet with parents alone or make academic accommodation or academic decisions without the student’s participation and consent. This policy aligns with federal privacy law (FERPA) and supports student self-advocacy.
If an appointment is made and the student is unable to attend, DSPS will cancel the appointment and reschedule at a later time when the student can attend.

THINGS TO NOTE FOR NEW AND CONTINUING STUDENTS:

- Appointment Accommodations: If any accommodation is needed for the counseling appointment, such as an ASL interpreter,
please let DSPS staff know when making the appointment, with enough time allowed to
secure the requested accommodation.
- Release of Information: A Release of Information authorizes information sharing only. It allows the individual or entity named on the release to receive student information, but it does not grant authority to make academic or accommodation-related decisions on the student’s behalf. All academic and accommodation decisions require the student’s presence and active participation in the interactive process.

Yearly DSPS Orientation - New Each Fall Semester

- DSPS Orientation takes place approximately one week before the fall semester begins. Please contact the front desk if you are a new student and want to make sure you are invited to the DSPS orientation.
